About applying for, renewing or replacing a personal information card
The Tasmanian Government personal information card is a form of photo ID containing your full name, date of birth and address.
You can apply for a card, update details like your name or address, or replace a card by filling in the application form (PDF, 189KB) and following the instructions.
When you update your address, you will receive a label that you can stick over the old address on your existing card.

What you need to apply, renew or replace
- complete an application form (PDF, 189KB) - the statutory declaration section must be signed in front of a Service Tasmania customer service officer.
- provide proof of identity
- have your photo taken
- pay the fee.
If you are under 18, your parent or guardian must visit the service centre with you to provide proof of their identity.
Can I apply, renew or replace?
There are no restrictions on who can apply for, renew or replace a personal information card.
If you are under 18, you must visit a service centre with your parent or guardian.
Proving your identity
You must provide 3 documents from the following lists to support your application. All the documents must be:
- current
- the originals (not photocopies or certified copies)
- in English or provided with an English translation from an accredited translator.
- One document from category A and 2 documents from category B, or
- Two documents from category A and one document from category B
- If none of your documents show evidence of your current address, you will need to provide one document from category C.
- Tasmanian personal information card (can be expired up to 2 years)
- Australian birth certificate (not commemorative or extract)
- Australian citizenship certificate
- Australian passport (can be expired up to 2 years)
- Australian driver licence (can be expired up to 2 years)
- Australian firearm licence
- Overseas passport (can be expired up to 2 years if accompanied by a current Australian visa)
- Interstate photo ID card (can be expired up to 2 years)
- Department of Home Affairs travel documents or Australian Government issued Immicard
- Department of Home Affairs certificate of evidence of resident status.
- Medicare card
- Bank or credit card with your signature, issued by an Australian bank
- Student identity card photo issued by an Australian educational institution
- Concession cards issued by Centrelink or Department of Veterans' Affairs
- Working with Vulnerable People card
- Marriage certificate, change of name or deed poll (to prove change of name) issued by Births, Deaths and Marriages
- Visa Entitlement Verification Online (VEVO).
Category C (evidence of current residential address)
You must provide one of the following documents which must be less than 6 months old:
- Bank or financial institution statement
- Council rates bill
- Land tax valuation notice
- Electricity, water, phone, or gas bill (with evidence of payment)
- Lease or rent agreement
- Australian Taxation Office assessment notice.
If your name or address is incorrect on your licence or any documents you use to prove your identity, you must show proof of:
- your address, and/or
- change of name documents.
Change of address
To show proof of change of address you must provide one of the following documents:
- Bank or financial institution statement
- Council rates bill
- Land tax valuation notice
- Electricity, water, phone, or gas bill (with evidence of payment)
- Lease or rent agreement
- Australian Taxation Office assessment notice.
To show proof of change of name you must provide one of the following documents:
- Change of Name Certificate issued by BDM
- Marriage Certificate issued by BDM (not a commemorative or issued by a celebrant/church)
- Amended Birth Certificate (with notations) issued by BDM
- Deed Poll issued prior to 01/02/2004
- Divorce papers Decree Nisi or Absolute issued by a court may also be acceptable providing they show the name being reverted to.

- Your card will be sent to you within 15 working days of the application being lodged at a service centre. If your card has not arrived within 20 working days, please visit a service centre as soon as possible to arrange a replacement.
- To update your address please visit a service centre with your personal information card. A sticker will then be sent to your postal address for you to attach to the card.
