About applying for, renewing or replacing a personal information card

The Tasmanian Government personal information card is a form of photo ID containing your full name, date of birth and address.

You can apply for a card, update details like your name or address, or replace a card by filling in the application form (PDF, 189KB) and following the instructions.

When you update your address, you will receive a label that you can stick over the old address on your existing card.

If you are under 18, your parent or guardian must visit the service centre with you to provide proof of their identity.

There are no restrictions on who can apply for, renew or replace a personal information card.

You must provide 3 documents from the following lists to support your application. All the documents must be:
